I hear  ET a lot, but I have a different set of criteria. For me, the game must be playable. so, that drops ET for me (because its just too damn buggy). I will say the worst game I've ever played belongs to SuperMan 64, It nearly fails on every premise. The worst game I've played in the past 5 years: Real Time Conflict -- Shogun Empires (DS), because NO Strat should be beaten in less than a hour. EVER.

Honorable Mention -- Pac-Man (Atari version)
