I currently have a Geforce 8400 GS 256mb video card. I am looking to upgrade to a better card, but I honestly have no clue what to get. I am looking to spend around $50-$150 and would like a good to very good card.  I have both PCI-X and PCI slots on my pc.  I would be willing to buy a used card if it was in good condition, but would prefer new. If anyone has any suggestions I would greatly appreciate it.

You want to go pci-x

In the price range you can probably get like 1 250 or 260 gtx around 150-175 on sale etc. 

Just watch getting like the gts cards I prefer to stick with GT or GTX cards.   Try an get 512+ but go with a higher end card over going with alot of ram.   EG high end 512 vs a lower end card with 1g ram.

I should also probably pick up another fan as I have a slot in my case for one, but have never really needed to use it. Anyone have an extra they would be willing to part with somewhat cheap? I know they were often prizes at lans and I cant see paying too much to buy one new.

Looks nice but I prefer nvidia over ati anyday.   I think that is the one that Action got but ended up changing to an nvidia card but not sure if its the same model.   I know he got ATI then had to change to nvidia.

I had the 4870x2... nice card when it worked.  It had all kinds of artifacting, etc (like it was over heating) but even did it at boot... not sure if it was a conflict with my MB or not but returned it and got a Nvidia 275 which is almost as fast and sometimes faster anyway and cheaper too.  Also that 4870x2 when it was working was extremely loud.  I can not even hear the 275 even after hours of gaming.  So I highly recommend nVidia at this point.

This was Renegade's initial suggestion. After comparing the reviews of the 4870 and this I went ahead with this.

I also upgraded my 400w power supply to this 650w.
